Service Waivers

Please click the below link relevant to your scheduled service, fill out the waiver entirely and submit it before we begin your appointment.

Instructions:
-choose the correct artist who will be preforming your service
-enter the correct date for your scheduled service
-include your full legal name and a clear photo of valid government issued photo ID

For minors:
-the form is to be filled out for the person getting pierced, not the guardian
-include the full legal name of the person getting pierced and the guardian (in the spot provided below)
- include a clear photo of valid government issued photo ID of the person getting pierced and the guardian

If any of these are incorrect we will ask you to redo the waiver as it is for insurance purposes.
